The Federation of American Hospitals (FAH) offers four categories of membership: institutional, affiliate, associate and individual student.

Institutional membership criteria must include accreditation by the Joint Commission on Accreditation of Healthcare Organizations (JCAHO) or the American Osteopathic Association (AOA), or by Medicare or Medicaid certification, and duly licensed to operate in the state in which it is located. The affiliate membership category is "by invitation of the FAH Board of Directors" for leading health care companies that share the FAH philosophies and goals. Associate membership consists of the following categories: hospital associations, medical societies and foundations; suppliers (other than management companies) of services and/or products to the health care industry; and, management companies that do not own or manage hospitals but instead have contracts limited to certain functions, services, or areas of the hospital.
